It still happens. Each of the 2 or 3 times it's happened to me i was using a T-wrench. I typically use a T-wrench for everything. The AL threads will weaken over time...
Edit: And more importantly, look at how much thread engagement there actually is into the lower manifold...not very much which is also part of the problem. If they would have added a few more threads I'd agree with you (maybe).
